Not all Medicaid members are created equal when it comes to closing gaps in their care. Some will naturally attend care appointments without a second thought; others simply can’t due to transportation, child care or other restraints. Then there are members who are likely to close gaps with sufficient incentives and engaging reminders—and have the biggest impact on your HEDIS® score— if only you can reach them.

So how do we engage the hard-to-reach? In this case study, we partnered with a large Medicaid plan to do just that, simultaneously closing gaps in care and improving plan performance. Read on for our game plan.

Solution

At NovuHealth, we work with Medicaid plans to drive continuous engagement among the right member populations, so we can impact behavior cost-effectively. And as the data geeks that we are, we follow the research. In this case, the research showed us we’d be most successful by targeting those due to attend prenatal and well-child visits. So, we zeroed in on this likely-to-engage population, designing a rewards and engagement program to boost quality scores and better manage risk.

Our program specifically focused on the historically non-compliant, non-responsive members—those likely to impact HEDIS® scores. To drive engagement among these members and reach them wherever they might be, we developed an integrated multi-channel member experience (with a heavy focus on mobile), sent consistent communications and offered multiple, timely incentives to engage members throughout the program.

Ultimately, with the barriers Medicaid members often face when it comes to participating in the health system, we know it’s crucial to design a program that’s rewarding, simple and easy to use. Using extrinsic value to overcome barriers is key, as is reaching the right member with the right message via the right channel. We’ve woven these key findings throughout our Medicaid rewards and engagement programs, improving HEDIS® scores as we perform that ultimate health engagement feat: effectively reaching the hard-to-reach.
